N: 1. Moyamoya disease was first described in 1957 by Japanese surgeons Takeuchi and Shimizu. The term “moyamoya” means haze, mist, cloud, smoke or vapor. Moya is もや in Japanese, in Kanji (漢字), it is 靄, i.e. 霧靄 in Chinese, which means haze, mist or fog.

2. It is used to describe the angiographic appearance of the smoke-like abnormal vascular collaterals arising as a result of the stenosis. It occurs more in Orientals than in Caucasians. This condition may cause both cerebral ischemia and hemorrhage. Transient ischemic attacks may occur, and it is said that attacks are easier to occur after crying, blowing on hot noodles or playing wind instruments because of hyperventilation-associated cerebral vasoconstriction. There is no medical therapy. Bypass surgeries (direct or indirect) may help.

3. Moyamoya disease is a disorder of the arteries of the brain that produces narrowing of these conduits restricting the amount of blood that gets to the brain.

4. Moyamoya disease is part of a spectrum of diseases of the arteries of the brain named intracranial arterial stenosis. Stenosis means narrowing. If the arteries of the brain, the structures that bring blood with oxygen and nutrients to the brain, are narrow the amount of blood reaching the brain may be insufficient and this produces strokes.

5. Cultural Interrelation: We can mention episode 14 (season 6) of The Good Doctor. Drs. Alex Park and Shaun Murphy treat three-year-old stroke victim Nathan —a harrowing tale that Lee says “captures the essence” of what his family went through (Lee’s son, Cash, has moyamoya disease).
